2016-17 School Year Mandatory Benefits Enrollment
Enrollment in both supplemental insurance products and medical insurance is MANDATORY (Under ACA all employees must either enroll in or waive medical Insurance. There are also several mandated notices employers must provide employees each year. In order to comply with these two requirements, the district requires an active MANDATORY enrollment process each year from each employee.)

ENROLLMENT/DECLINATION FOR MEDICAL INSURANCE

All employees must either enroll or waive medical insurance coveage:
- Employees may see the UnitedHealthcare representatives during open enrollment to enroll in/waive medical coverage and complete a paper application. Representatives will be at each campus/department beginning May 23, 2016.
- Going to the Employee Benefits page on the district’s website and retrieving the form, completing it & submitting it to the Employee Benefits Department.

All employees must either enroll or waive medical insurance coveage:
- ASSISTED ENROLLMENT SESSIONS – Assisted enrollment session will be held June 6-9 and June 13-16 from 9:00 AM to 4:00 PM Monday through Thursday at the Watts Elementary Computer Lab for those employees who wish assistance in enrolling. You will be able to enroll in both the Supplemental Insurance products and the UHC medical insurance.
Employees have the option of completing the online process on their own at their leisure, or, employees may attend one of the assisted enrollment sessions to be held at Watts Elementary this year in the computer lab. Please use the side door for entry as the front of the building is being renovated.
Insurance representatives will be available at the dates and times below to assist employees in their choices and to answer questions.

June 6 9:00 am to 4:00 pm June 15 9:00 am to 4:00 pm
June 7 9:00 am to 4:00 pm June 16 9:00 am to 4:00 pm
June 8 9:00 am to 4:00 pm June 17 9:00 am to 4:00 pm
June 9 9:00 am to 4:00 pm
The benefit enrollment process is mandatory even if you choose not to enroll in district benefits. If you do not wish to participate in any district benefits, you must still complete the demographic information and acknowledge the mandatory notices. View and then decline all benefits to complete the mandatory enrollment process.
To prevent delay in your benefits coverage, please complete the online enrollment process. Existing coverage will not be rolled over to the 2016-2017 school year.
